Job Description

The Social Navigator opens doors of opportunity for individuals and families to connect with resources, services and programs they need to improve social determinants of health.  Individuals and families rostered to Lu’ma Medical Centre are introduced to the Social Navigator for support onsite and referrals to community service providers.  The Social Navigator has a blend of appointments, spontaneous introductions, and one to one sessions.  You may facilitate &/or participate in occasional workshops for patients on site.  You have the ability to respond to crisis as needed.  The Social Navigator lead delegates referrals and you are able to responsibly handle confidential files, documents, support patients in completing forms, and make submissions of documents as needed.

 

Responsibilities

Responsibilities include being an active listener, listening to the client’s goals, assisting with social determinants, assist in completing government forms, responding to crisis, charting, maintaining records of completed work in EMR, attending interdisciplinary clinical/case conferences, connecting clients with community services, programs and resources, liasing with hospital & healthcare centres, and advocacy as needed. In the Social Navigator role you will:

·Maximize the functioning and independence of clients through system navigation support to individuals, families and groups, and maintaining necessary records;

·Ensure clients receive community supports, health, social, housing, and recreational services to which he/she is entitled by acting as an advocate;

Introduce patients to other Lu’ma Team members as needed for appropriate services or support;

·Act as a resource person to Team members and community residents by compiling information on community resources and facilitate access to those resources by liaising with community groups;

·Seek out and research new services and resources in the community for patients to access;

·Liase with Hospital social workers to provide seamless services for a community of caring after discharge;

·Ensure comprehensive treatment for clients by providing information to other Team members on an ongoing basis, and by providing consultation as needed;

·Assist individuals complete applications for access to resources people qualify for such as: PWD, CPPD, EI, Subsidies, GST, etc.

·Assist individuals complete applications for ID:  Status, Birth, SIN, Name change, Drivers License, Passport etc.

·Support families who are navigating VACFSS or MCFD systems;

·Participate in weekly Social Navigator team meetings and other case management meetings;

·Ensure all client records, assessment notes and correspondence are kept organized, secure, & have remain confidentiality;

·Chart necessary information in OSCAR EMR as required in a format that follows SOAP note and LMC conventions;

·Follow protocols and policies as required by LMC;

·Follow-up contact with clients to ensure the services you have coordinated have been connected appropriately;

·Provide leadership for appropriate community connections for the team and patients at Lu’ma Medical Centre; and

·Supervise social work students in field placement practicums.

·Participate in program evaluation/quality improvement by identifying opportunities for improvement in services. 

·Participate / witness respectfully in cultural protocols and ceremonies with clients and team.

 

SKILLS AND QUALIFICATIONS

·        Demonstrated experience providing services to individuals with lived experience;

  • Minimum of two years of experience in a community-based service organization. Thorough knowledge of and proficiency in current assessment and social work techniques.

  • Thorough knowledge and understanding of community resources in Greater Vancouver.

  • Demonstrated ability to deal effectively with crises.

  • Excellent communication and interpersonal skills.

  • Demonstrated ability to work in a multi-disciplinary team environment.

  • Proficiency and experience working in a computerized environment.

  • Cultural safety awareness and experience.

  • An understanding of the British Columbia Mental Health Act, Mental Health reform principles, the Substitute Decisions Act, Jordan’s Principle, and the Health Care Consent Act and relevant privacy legislation at both the provincial and federal levels.

  • Working knowledge of FNHA Benefits and ability to connect clients with eligible funding sponsors such as Social Assistance Benefits.

  • Successful candidates must undergo a Vulnerable Sector Screening reference check to the satisfaction of Lu’ma Medical Centre management.

  • Indigenous ancestry to turtle island is an asset and a preference.

 

Knowledge and Abilities

You have the demonstrated ability to:

· Communicate effectively both verbally and in writing with clients, families, team members and community agency personnel;

·  Organize and track active work files in progress, organize and maintain files, chart, and submit documents to appropriate agencies;

·  Develop trusting and respectful relationships with team members and people you serve together;

·        Be curious, objective and compassionate, not judgemental;

·        Resolve and de-escalate conflict situations;

·        Work collaboratively as a member of an interdisciplinary team.

·        Operate a laptop and use Microsoft Office and EMR.

·        Physical ability to perform the duties of the position.

·      Self regulate oneself mood and emotions in stressful situations and maintain     professional composure and communication with others.

·        Daily engage in self-wellness both at work and at home.

·        Create space for culturally safe care.

 

Educational Requirement

  • Bachelors in Social Work from a recognized university and extensive experience in supporting individuals and families navigate social service systems;

  • Current member in good standing the British Columbia College of Social Workers.

·       Two (2+) years of recent, related experience in a primary care setting and/or community healthcare setting.

·       Registration with a regulatory body - In good standing.

Job Summary

The Trauma & Addictions Counsellor provides one-to-one counselling and group work with people on our patient panel. Your skills will help create dynamic supportive groups to meet rising needs among our patient panel such as coping with anxiety, PTSD, addictions and depression.  Your scope of practice will support patients on their healing journey. You may from time to time be asked to participate in health workshops by facilitating a mental health educational workshop for participants.  At times, you will work collaboratively with other members of the mental health team and multi-disciplinary team to care for patients.

Responsibilities

  • In the Trauma & Addictions Counsellor role you will:

  • Provide Counselling sessions with individuals;

  • Facilitate group counselling. (Topics: Trauma, Anxiety, Depression, Grief & Loss, Addictions);

  • Prepare and provide a Counselling treatment plan;

  • Have exceptional and professional communication skills;

  • Provide Counselling services within the guidelines of the program;

  • Participate in clinical counselling meetings with colleages; and

  • Chart and make Clinical Counselling notes in the EMR.

  • Support individuals who would like to apply to Treatment Centres with the application process.

 Knowledge and Abilities

You possess the following skill set:

  • Exceptional facilitation skills and experience in Clinical Counselling for groups in a healthcare facility;

  • Training and experience in professional therapeutic counselling modalities;

  • Experience in addictions counselling;

  • Knowledge of urban Indigenous challenges and barriers and the history of Indigenous people in Canada;

  • Ability to provide competent and culturally safe care;

  • Ability to balance traditional and western approaches in practice.

  • Excellent computer literacy to utilize an EMR information system (OSCAR an asset), Zoom, Microsoft Office, Internet and email software; and

  • Demonstrated ability to work independently, supportively with Counselling colleagues, and on occasion collaboratively with an interdisciplinary team.

At our primary care centre you will be working in a shared care model with:

A multi-disciplinary team of Counsellors, Registered Nurses, Medical Office Assistants, Traditional Elders and Healers, Social Navigators, Family Medical Practitioners and Practicum Students.

Qualifications & Experience:

  • A Masters of Social Work major with practicum in Counselling or Masters of Clinical Counselling

  • Must be Registered as a Clinical Counsellor with a professional regulatory body in BC: BCACC or BCASW

  • A wide scope of practice & training (Trauma, Addictions, Somatic, CBT, EMDR, DBT, AFOT, MI, PT, Five (5+) years of recent, related experience Counselling in a community mental health or healthcare setting; and

  • Five (5+) years of experience working for an organization with an Indigenous mandate.

  • Proficiency and accuracy in using Word, Excel and Outlook;

  • EMR (Electronic Medical Record) system proficiency is an asset;

  • Ability to conduct healthy working relationships with clients, guests, team, patients, elders and community;

  • Ability to physically, emotionally, and mentally carry out the duties of the position;

  • Ability to conduct comprehensive biopsychosocial spiritual assessments in accordance to professional standards;

  • Ability to work independently, demonstrating motivation;

  • Strong communication skills and ability to support and collaborate in a team of multi-disciplinary practitioners

  • Demonstrated working knowledge of applicable legislations ie. Mental Health Act, Child & Family & Community Services Act, Guardianship Act…

  • Demeanor that is kind, compassionate, culturally safe, and empathetic.

  • Comprehensive knowledge of Indigenous culture, protocols, history in a demonstrated knowledge of the legacy of colonization;

  • Understand privacy and ability to maintain professional confidentiality;

  • Participation in continuing professional development to remain current; and

  • Knowledge of various counselling theories and therapeutic modalities.

Building Service Worker

Lu’ma Native BCH Society is seeking to fill two Building Service Worker positions on a Full-Time basis at our supportive housing programs for the DAY shift; Monday to Friday: 8:30am until 5:00pm.

 Position Summary:

Under the direct supervision of the Program Manager, the incumbent will be responsible for the general maintenance, cleanliness, and upkeep of the supportive housing programs located in Vancouver and North Vancouver. 

 Key Duties and Responsibilities:

·        Maintain floors; vacuuming, washing, stripping, waxing, mopping and/or shampooing using various power and hand cleaning equipment;

·        Maintain cleanliness of toilets, shower rooms, kitchen and laundry rooms;

·        Assist with residents’ laundry and suites as needed;

·        Collects and disposes of refuse and maintains clean refuse areas;

·        Dust, wipe and polish windows, and other fixtures and furniture; perform spot cleaning as required;

·        Maintain the cleanliness of the immediate grounds, including sweeping the front sidewalk and walkways;

·        Check stairs routinely and clean as needed;

·        Arrange furniture for special events;

·        Maintain various logs, janitorial supplies and other records.

·        May assist in taking inventory.

·        Minor maintenance and other duties as may be assigned by the Program Manager or Director of Supportive Housing.

·        Perform additional duties as requested and/or required.

 Job Skills & Abilities:

·        WHMIS certificate

·        Physical ability to carry out heavy cleaning and ability to lift to 23 kg (50 lbs.);

·        Ability to operate heavy industrial grade cleaning equipment;

·        General maintenance skills;

·        Demonstrated ability to be organized and carry out work with independence, and the ability to work cooperatively and effectively under general supervision;

·        Ability to work effectively with program staff, community partners and residents, including being non-judgmental in trying situations;

·        Ability to give and receive constructive criticism;

·        Thorough knowledge of community and building issues; and understanding of standards of maintenance, CoV Bylaws, WorkSafe BC processes, the Residential Tenancy Act and other applicable regulatory requirements.

 Remuneration and Hours:

·        Full Time position, Tuesday to Saturday; 8:30am until 5:00pm, 8.5 hours per day with a half hour unpaid lunch break.

·        Wage is $28.61 per hour; plus a generous Benefits package, RRSP match to 3.5%; Statutory and Paid holidays (Article 10 of the Collective), Vacation and other paid leaves will be awarded on a pro rata basis.

Casual Tenant Support Worker

Lu’ma Native BCH Housing Society is seeking to fill the position of Tenant Support Worker on a Casual On Call basis at our Supportive Housing sites located in Vancouver and North Vancouver for All Shifts: 7:30am until 4:00pm, 3:30pm until 12:00am, and 11:30pm until 8:00am.

Under the direct supervision of the Program Manager, the Tenant Support Worker provides non-judgmental support, advocacy and resource information to residents. 

Key Responsibilities:

Familiarizes new residents with the various program sites including the physical environments and guidelines for living communally and works to facilitate positive relationships between residents;

Provides information regarding options and alternatives including information about income assistance, legal services, health and mental wellness issues, drug and alcohol services, housing and other resources as requested;

Provides one-to-one supports to residents

Participates in supporting residents to meet their goals and objectives;

Maintains open, honest and direct communication with residents and former residents;

Provides group support, including facilitation of conflict resolution between residents and groups of residents;

Responds to building emergencies.

Qualifications/Skills & Abilities:

A degree, diploma or certificate in a related subject and/or relevant skill-based training;

Minimum one-year experience working as a front-line support worker with people who have complex needs and who are struggling with poverty and homelessness;

Must be open to working with people who are struggling with substance use and/or mental & spiritual wellness;

Must understand and be comfortable working within a Harm Reduction framework;

Ability to work effectively with program staff, community partners and residents, including being non-judgmental in trying situations;

Ability to work under pressure and manage stresses and crisis is required;

A valid Occupational First Aid level 1, Overdose Prevention, Non-Violent Crisis Intervention, Food Safe level 1 and WHMIS certificate is required;

All positions are subject to a Vulnerable Sector Criminal Records review

 Hours and Remuneration:

Casual position, 8.5 hours per day with a half hour unpaid lunch. Shifts are: 7:30am until 4:00pm; 3:30 pm until 12:00 am; and 11:30pm until 8:00am the following day (Overnight awake shift).  

Wage is $28.61 per hour plus 18% in lieu of Statutory and Paid Holidays as per Article 8 in the Collective Agreement, Annual Vacation Pay, Leaves of Absence (Article 10), Pension and Benefits. The hourly rate inclusive of in lieu is $33.76.
