About the North Vancouver Complex Care Housing team

Lu’ma BCH Housing Society is launching new Complex Care (CCH) outreach services for First Nations and urban Aboriginal residents living with acute, complex, and concurrent medical conditions in our transitional and modular housing projects in North Vancouver. We are building a core team of inter-disciplinary Indigenous and culturally aware practitioners to lead and deliver care together in a culturally safe, wholistic, integrated way. Within the context of a client centered care model and in accordance with established standards of professional practice, and vision and values of culturally integrated care, the Cultural Outreach Worker works as an embedded team member in the complex Care Housing Team to provide culturally safe services, supports and guidance to participants in the program.

 

About the Cultural Outreach Worker Role

You work to address client centred and culturally safe care supported by an interdisciplinary primary care outreach team. Under the direct supervision of the Team Lead, you work as a member of the team to facilitate increased access to culture and cultural knowledge for participants and team members. You coordinate access to Elders, including the team Elder, and Knowledge Keepers. You foster connections and collaboration with cultural supports and agencies in community.

 

Duties and Responsibilities

1.      Facilitates increased access to culture and cultural knowledge from a perspective that promotes healing for program participants.

2.      Act as a cultural resource and support as required.

3.      Facilitate Talking, Teaching and Healing Circles.

4.      Facilitate programs for cultural practices including traditional medicines, arts, and crafting.

5.      Supports the planning and running of Seasonal events.

6.      Connect with Elders and/or Traditional Healers, and resources to conduct ceremonies.

7.      Provide cultural advocacy on behalf of the participants with other individuals and/or community/government agencies by interacting with others to resolve concerns and/or issues of clients and providing emotional support to clients.

8.       Documents client care plans and disposition on the client file as appropriate.

9.      Collates and generates statistical information as requested by the Team Lead.

10.  Performs other related duties as required.

Education / Qualifications Requirement

Minimum of 2 years of work experience in an Indigenous oriented environment

Occupational First Aid Level

Non-violent Crisis Intervention Certificate

Food Safe Level 1 Certificate

Valid Class 5 Drivers License with access to a reliable vehicle

An equivalent combination of training and experience may be considered.

Experience, Working Knowledge and Abilities

Knowledge of community resources and services. Knowledge of Indigenous culture and world views. Working knowledge of the social emotional and health-related problems encountered by the population of the downtown community eastside. Demonstrated ability to work under pressure, to work independently with minimal direction, to problem solve, to take initiative, to anticipate potential problems/conflicts and take appropriate actions, to organize and prioritize work, and to meet deadlines. Demonstrated ability to establish satisfactory work relationships with the public and other employees, to deal tactfully and effectively with co-workers, public and staff from other agencies, to maintain confidentiality and to communicate effectively both verbally and in writing. Exceptional customer service and communication skills. Ability to work within a team environment.

Salary: $66, 747 per annum

Hours: 35 hour week; Monday to Friday 8:30am to 4:30pm. (7 hours paid with a 1 hour unpaid lunch break)

Benefits: Employer Paid Extended Medical and Dental, LTD, Critical Illness and Life Insurance coverage and Optional RRSP with an Employer match to 3.5% after 90 days.

Vacation: Vacation is 6% of gross and is accessible after 1 year.
