About the Tenant Support Worker Role
Lu’ma Native BCH Housing Society is seeking to hire Tenant Support Workers on a Casual-On Call basis at our supportive housing sites. Must be willing to work Saturdays, Sundays and Statutory Holidays. Under the direct supervision of the Program Manager, the Tenant Support Worker provides non-judgmental support, advocacy and resource information to residents.
Responsibilities
Familiarize new residents with the various program sites including the physical environments and guidelines for living communally.
Understand program information and direct resources to residents including but not limit to income assistance, legal services, health and mental wellness issues, drug and alcohol services, housing.
Advocate for residents with external agencies and services.
Conduct resident wellness checks, arrange medical appointments, and accompany residents as required.
Provide one-to-one and group support to residents to meet their goals and objectives and facilitate conflict resolution between residents.
Maintains open, honest and direct communication with residents and former residents; and work to facilitate position relationships between residents.
Respond to room/building emergencies as needed.
Qualifications
Require:
A degree, diploma or certificate in a related subject and/or relevant skill-based training.
Valid Occupational First Aid Level 1 (or equivalent).
WHMIS.
Food Safe Level 1 (or equivalent).
Overdoes Prevention.
Non Violent Crisis Intervention/MANDT.
Mental Health First Aid.
Naloxone.
Minimum 1 year experience working as a frontline support worker with people who have complex needs and struggle with poverty and homelessness.
Open to work with people with issues of substance use and/or mental wellness.
Understand and be comfortable working within a Harm Reduction framework.
Ability to work effectively with program staff, community partners and residents, including being non-judgmental in trying situations.
Ability to work under pressure and manage stresses and crisis.

Duties and Responsibilities
Responsibilities include listening to the client’s goals, assisting with social determinants, assist in completing government forms, crisis counselling services, charting, maintaining records of completed work in EMR, attending interdisciplinary clinical/case conferences, connecting clients with community services, programs and resources, liaising with hospital & healthcare centres, and advocacy. Working as a professional and being a care team contributor.
1. Listens to the client’s healthcare experience, conducts social work needs assessments through conversation with client. Identifies the client’s coping abilities and provides client with self-regulation tools and skills in cases of complex social emotional issues.
2. Provides trauma and crisis counselling by exploring the emotional response to identified problems, personal concerns and treatment objectives to facilitate change in coping style, behaviour, attitude, feelings and adaptive responses to an adverse life event. Provides referrals for long term counselling as necessary.
3. Attends case conferences as a member of an interdisciplinary team for client reviews, admissions and discharges by providing input from a social work perspective on a variety of care issues related to the client to ensure proper and effective treatment of the client.
4. Provides consultative and direct services to clients, team members and community health professionals.
5. Understands professional guidelines of confidentiality, conflict of interest, and has developed ethical and moral values.
6. Maintains client records by charting encounters, client goals, assessments, interventions, modifications, appropriate client information, progress reports, and scanning in documents and files to the EMR. Charts SOAP notes in accordance with established standards, policies and procedures in compliance with regulatory requirements.
7. Maintains a basic record of workload statistics by recording information into approved formats, and collating data.
8. Participates in and/or leads meetings and case conferences.
9. Participates in program evaluation/quality improvement by identifying opportunities for change and formulating recommendations to better improve social work services.
10. Participates / witnesses respectfully in cultural protocols and ceremonies with clients and team.
11. Performs other related duties, as assigned.
Qualifications Requirement
Current full registration with the British Columbia College of Social Workers. Bachelor’s degree in Social Work from a qualified education provider recognized professionally in BC. Training in Non-violent Communication, Trauma-informed Care, and De-escalating Conflict Valid BC Driver’s License and access to a personal vehicle for business-related purposes.
Experience, Working Knowledge
Two (2) years’ recent experience working with First Nation’s community members with concurrent health issues, mental health and substance use issues. Knowledge and interest in culturally integrating care with team members. Knowledge of local (i.e. Downtown Eastside) and regional initiatives, services, programs and resources that may accessed for client care, Working knowledge of other health disciplines and their role in a shared care team. Working knowledge of legislation and government policies and procedures that relate to the client. Experience and practice in trauma and addictions counselling.

Within the context of a client centered care model and in accordance with established standards of professional practice, and vision and values of culturally integrated care, the Peer Outreach Worker works as an embedded team member in the complex Care Housing Team to provide wholistic care and individual peer outreach support from lived experience.
About the Peer Outreach Worker
The Peer Outreach Worker is a person who self-identifies as a Peer, meaning a person with lived/living experience relevant to the client group served. Under the direct supervision of the Team Lead, you work as a member of the interdisciplinary team to provide a variety of advocacy/liaison services to clients within the designated program living with health issues, screen clients for eligibility for services, link/accompany clients to appropriate internal and external resources and advocate for clients with various community and/or government agencies. You help facilitate healthy professional working relationships based on trust, respect and healthy boundary setting. You have excellent communication skills with people you serve, team members, and stakeholders. You ensure and support a high standard of care and promote integrated collaboration amongst the team, hospitals, healthcare centres, and community service providers. You are responsible for reporting weekly activities, stats, updates, and resource needs to the Team Lead.
Duties and Responsibilities
1. Provide advocacy and/or liaison services on behalf of the designated program clients with other individuals and/or community/government agencies by interacting with other Health professionals to resolve concerns and/or issues of clients and providing emotional support to clients.
2. Identifies and prioritizes the non-medical problems of clients and provides conflict resolution as required.
3. Screens/assesses clients to establish eligibility or appropriateness for eligibility to external health care services and housing. Books appointments for clients when appropriate.
4. Serves as an information resource regarding available community services for clients and staff. Identifies and locates community resources that meet the needs of all clients within the program. Refers clients to alternative organizations and programs as appropriate.
5. Assists the client in the preparation of submissions and/or documents to government and other agencies.
6. Ensures that clients are oriented to the designated program services.
7. Documents client care plans and disposition on the client file as appropriate.
8. Performs a variety of general clerical duties such as responding to calls/answering inquiries regarding routine program information, maintaining client files, completing disability and housing forms, utilizing a variety of computer software applications produces a variety of documents, filing, photocopying, faxing and data entry.
9. Collates and generates statistical information as requested by the Team Lead.
10. Performs other related duties as required.
Education / Qualifications Requirement
Minimum of 2 years of support work experience in a supportive housing or healthcare oriented environment; Lived experienced that is relevant to the Indigenous community members we serve in the Complex Care Housing Model; Occupational First Aid Level 1; Non-violent Crisis Intervention Certificate; An equivalent combination of training and experience may be considered.

About Team Lead Role
Under the direct supervision of the Director of Health Services and the Director of Supportive Housing, the Team Lead will have a passion for delivering team based healthcare delivery solutions to First Nations and urban Aboriginal peoples. You collaborate with the interdisciplinary outreach team members to deliver client centred and culturally safe primary care. You coordinate and support the delivery of daily activities of care for residents of transitional & supportive housing projects. Duties and Responsibilities
Supports client’s goals and multi-disciplinary team perspectives on wholistic healthcare solutions through an Indigenous lens of wellness. Facilitates supporting change and improved client healthcare experiences and client determination of wellness. Facilitates excellent team communications, rounds, and interdisciplinary clinical/case conferences. Ensures excellence in culturally safe and culturally integrated primary, complex, mental health care for clients. Ensures clients have access to community of care through connections to community services, programs and resources. Assists team in liaising with hospital & healthcare centres, and advocacy, Working as a professional, care team supporter, and wellness initiator. Ensures team and client encounters, progress, forms, tests, assessments, consent forms, etc are being charted and or scanned to the EMR and faxed to appropriate primary care MRP’s. Responsible and accountable for reporting client and team achievements, performance stats, and data.
Education / Qualification Requirements
Master’s degree in Allied Health discipline from a recognized program. Five (5) years recent related experience providing and leading primary care, mental health, and addictions clinical services. Training in Non-violent Communication, Trauma-informed Care, and De-escalating Conflict Cultural safety training Valid BC Driver’s License and personal vehicle required. Clear criminal record check to work with vulnerable peoples.
Experience, Working Knowledge
At least two (2) years’ experience working with First Nations community members with concurrent health issues, mental health and substance use issues. Comprehensive knowledge of mental illness and addiction issues including current trends, best practices and modalities of treatment. Broad knowledge of mental health and addiction service delivery system, service mandates, referral guidelines, legislation and government policies. Knowledge of how Indigenous community members continue to be impacted by generations of colonization, trauma, systemic racism and discrimination, and as a result disproportionately experience mental health and substance use challenges and homelessness. Broad knowledge of traditional Indigenous approaches to wellness, primary care and mental health modalities.

Encampment Outreach Worker - Complex Care Housing Team
About the Complex Care Housing team:
Lu’ma Medical Centre is launching new Complex Care (CCH) outreach services for First Nations and urban Aboriginal residents living with acute, complex, and concurrent medical conditions in four transitional and modular housing projects. We are building a core team of inter-disciplinary Indigenous and culturally aware practitioners to lead and deliver care together in a culturally safe, wholistic, integrated way. Within the context of a client centered care model and in accordance with established standards of professional practice, and vision and values of culturally integrated care, the Outreach Worker works as an embedded team member in the complex Care Housing Team to provide wholistic care and individual outreach support from a cultural lens.
About the Encampment Outreach Worker:
The Encampment Outreach Worker will be responsible for outreaching to encampments and will require the successful applicant to feel comfortable working in these settings. Under the direct supervision of the Team Lead, you work as a member of the interdisciplinary team to provide a variety of advocacy/liaison services to clients within the designated program living with health issues, screen clients for eligibility for services, link/accompany clients to appropriate internal and external resources and advocate for clients with various community and/or government agencies. You help facilitate healthy professional working relationships based on trust, respect and healthy boundary setting. You have excellent communication skills with people you serve, team members, and stakeholders. You ensure and support a high standard of care and promote integrated collaboration amongst the team, hospitals, healthcare centres, and community service providers. You help evaluate services to ensure program/service needs are met and liaise with key stakeholders on the achievement of program goals and objectives. You are responsible for reporting weekly activities, stats, updates, and resource needs to the Team Lead.
Duties and Responsibilities:
1. Supporting positive relationships with community members residing in homeless encampments.
2. Working with Indigenous community members who are sheltering outside to provide cultural supports.
3. Coordinating elders and ceremonies as needed
4. Practicing from a person-centred approach by connecting community members with preferred resources if needed.
5. Lead meetings as required with participants
6. Attend weekly meetings with stakeholders
7. Build relationships with service providers in order to best support the needs of those sheltering outside
8. To work in collaboration with the Vancouver Board of Parks and Recreation to ensure that work with those sheltering outside is conducted through a rights-based lens.
9. Provide advocacy and/or liaison services on behalf of the designated program clients with other individuals and/or community/government agencies by interacting with other Health professionals to resolve concerns and/or issues of clients and providing emotional support to clients.
10. Identifies and prioritizes the non-medical problems of clients and provides conflict resolution as required.
11. Screens/assesses clients to establish eligibility or appropriateness for eligibility to external health care services and housing. Books appointments for clients when appropriate.
12. Serves as an information resource regarding available community services for clients and staff. Identifies and locates community resources that meet the needs of all clients within the program. Refers clients to alternative organizations and programs as appropriate.
13. Assists the client in the preparation of submissions and/or documents to government and other agencies.
14. Ensures that clients are oriented to the designated program services.
15. Documents client care plans and disposition on the client file as appropriate.
16. Performs a variety of general clerical duties such as responding to calls/answering inquiries regarding routine program information, maintaining client files, completing disability and housing forms, utilizing a variety of computer software applications produces a variety of documents, filing, photocopying, faxing and data entry.
17. Collates and generates statistical information as requested by the Team Lead.
18. Performs other related duties as required.

Education / Qualification Requirements:
Minimum of 2 years of support work experience in a supportive housing or healthcare oriented environment; Lived experience that is relevant to the Indigenous community members we serve in the Complex Care Housing Model is preferred; An equivalent combination of training and experience may be considered.
-Occupational First Aid Level 1;
-Non-violent Crisis Intervention or MANDT Certificate;
-Valid Class 5 BC Driver's License
-Naloxone training

Hours and Remuneration:
Hours of work are 8:30 am to 5:00 pm, Monday to Friday. Schedule may be varied to suit the needs of the program; where possible, 2 weeks’ notice will be provided. This a temporary position with an expected end date of March 31, 2025.
Rate of Pay: $30.87 hourly plus 18% in lieu of Benefits, RRSP, Vacation, Statutory Holidays, and other paid leaves. Wage inclusive of the in-lieu amount is $36.42 hourly.

You have completed a BSW, have a passion for working with the First Nations away from home and urban Aboriginal community, experience doing social work in healthcare, experience in trauma and crisis counselling, and love working in a multi-disciplinary team. You are generous with your wide range of skills, tools, experience, and gifts to contribute to shared care and health plans. You genuinely enjoy relationship building with individuals you assist and have excellent professional communication skills working in a circle of caring. You have experience working with people suffering from concurrent mental and physical health issues. You are compassionate and genuinely interested in strength-based approaches and solutions. You are available to commit to working in-person 5 days per week 8:30am-5pm at the transitional housing programs located in the DTES Vancouver.
Duties and Responsibilities
Responsibilities include being an active listener, listening to the client’s goals, assisting with social determinants, assist in completing government forms, crisis counselling services, charting, maintaining records of completed work in EMR, attending interdisciplinary clinical/case conferences, connecting clients with community services, programs and resources, liasing with hospital & healthcare centres, and advocacy. Working as a professional and being a care team contributor.
1. Listens to the clients healthcare experience, conducts social work needs assessments through conversation with client. Identifies the client’s coping abilities and provides client with self-regulation tools and skills in cases of complex social emotional issues.
2. Provides trauma and crisis counselling by exploring the emotional response to identified problems, personal concerns and treatment objectives to facilitate change in coping style, behaviour, attitude, feelings and adaptive responses to an adverse life event. Provides referrals for long term counselling as necessary.
3. Attends case conferences as a member of an interdisciplinary team for client reviews, admissions and discharges by providing input from a social work perspective on a variety of care issues related to the client to ensure proper and effective treatment of the client.
4. Provides consultative and direct services to clients, team members and community health professionals.
5. Understands professional guidelines of confidentiality, conflict of interest, and has developed ethical and moral values.
6. Maintains client records by charting encounters, client goals, assessments, interventions, modifications, appropriate client information, progress reports, and scanning in documents and files to the EMR. Charts SOAP notes in accordance with established standards, policies and procedures in compliance with regulatory requirements.
7. Maintains a basic record of workload statistics by recording information into approved formats, and collating data.

Qualifications Requirement
Current full registration with the British Columbia College of Social Workers to practice.
Bachelors degree in Social Work from a qualified education provider recognized professionally in BC.

Building Service Worker
Lu’ma Native BCH Society is seeking to fill a Building Service Worker position on a Temporary Full-Time basis at our supportive housing program for the DAY shift; Monday to Friday: 8:30am until 5:00pm.
Position Summary:
Under the direct supervision of the Program Manager, the incumbent will be responsible for the general maintenance, cleanliness, and upkeep of the supportive housing program located in Vancouver.
Key Duties and Responsibilities:
· Maintain floors; vacuuming, washing, stripping, waxing, mopping and/or shampooing using various power and hand cleaning equipment;
· Maintain cleanliness of toilets, shower rooms, kitchen and laundry rooms;
· Assist with residents’ laundry and suites as needed;
· Collects and disposes of refuse and maintains clean refuse areas;
· Dust, wipe and polish windows, and other fixtures and furniture; perform spot cleaning as required;
· Maintain the cleanliness of the immediate grounds, including sweeping the front sidewalk and walkways;
· Check stairs routinely and clean as needed;
· Arrange furniture for special events;
· Maintain various logs, janitorial supplies and other records.
· May assist in taking inventory.
· Minor maintenance and other duties as may be assigned by the Program Manager or Director of Supportive Housing.
· Perform additional duties as requested and/or required.

· Wage is $28.61 per hour; plus 18% in lieu of Benefits package, RRSP match to 3.5%; Statutory and Paid holidays (Article 10 of the Collective), Vacation and other paid leaves. Wage inclusive of the in lieu amount is $33.76 per hour.
